The gap after discharge

Medically stable isn't the same as ready to go home.

A hospital stay can end before recovery does. Some patients are well enough to be discharged but still need wound care, medication management, or simply a safe place to rest — support that an empty apartment, a shelter, or the street cannot provide.

Without that safe next step, recovery stalls and the risk of a return trip to the emergency room goes up. New Era exists to close that gap.
